It's an 3x3 box template set
available at MFT here

I wanted to make a 3x3 box (my fave) with a very well fitting lid... that would have all kinds of accessories, so that it would be flexible for whatever you'd like to create. It's called 3x3 but technically it's 3 1/2 inches square (tailored to fit your 3x3 card creations & envelopes or treats or gifts...)
Here are 4 projects that I made with the box & accessories templates:
(and new MFT sets, of course!) ;O)
For me, 3x3 card sets are an ideal gift-y idea, since they are so affordable to
make & people can use the note cards to give away, but keep the box if they like.
This puppy has a gift card holder:
(The panels that come with it are
easy to emboss with folders or plates):
and it comes with liners to decorate the inside & hide all the tabs:
It can make a quick high-backed box:
...and has a box divider.
(If you use the divider in the lid it fits business cards perfectly)
It slices. It dices! And cleans in a snap! LOL
The envelope template (a double one for 2 3x3 envelopes at once)
has liners too (and measures 3 1/4" to allow for all the
good bits we like to add to cards) :o)
